My health, my community
What did we do?
As part of Pennine Care NHS Foundation Trust‘s ‘my health, my community‘ work, the focus of this project was on building a collaborative partnership between all agencies, patients, people and the community in order to establish a ‘virtual’ academy which promotes self-care, community resilience and patient and carer wellbeing.
Why was it important?
This work supported ‘my health, my community’, which aims to help people gain a greater understanding of long-term conditions so they can self-manage and provide much needed support to carers.
How did we do it?
In line with The Health Foundation’s use of ‘evidence scans’, we provided reviews of bespoke patient and carer educational programmes and tools to aid the management of end-of-life care across six areas: cardiovascular disease, stroke, chronic obstructive pulmonary disease, paediatric diabetes and general end-of-life care.
